You don't HAVE to smoke a cigarette to have a smoko! That is just its origin because so many people used to smoke. It is just the trad morning and afternoon tea break (increasingly now a coffee break) in a workplace. The term also extended to the name of the room where it used to take place in factories etc - the smoko room. And if you don't have a room at your workplace, this is what a smoko looked like in the outdoors, though I doubt that lunch was ever called a smoko:
