I recognize the 'first-pass' scenario near the same time period of meds - not that I would drink/eat it anyway. Actually a lot of the idea has been sorta debunked - but if one were to consume grapefruit a few times a week, the effect is cumulative. |

| 1158785 | 2010-12-08 06:03:00 | Elavil is amitriptyline ... an antidepressant & don't think grapefruit will have any affect on it ... however ... any opiate based painkillers like Morphine or Oxycodone is a different matter. Taking cough syrup with opiates also has a similar effect if it has Dextromethorphan as an ingredient. And yes ... I speak from personal experience after letting a surgeon have a play with my back. |

It interacts by inhibiting a large variety of enzymes belonging to the Cytochrome P450 family (CYP450, also known as). As a result, the activities of the key enzymes that metabolise majority of the drugs in the market such as CYP3A4, CYP2D6, CYP2C19 etc can be inhibited when grapefruit juice and / or its derivative are consumed, causing the plasma level of the drug to slowly increase until it is beyond the safe level. Personally, if I were asked to recommend a TCA antidepressant, I would stay away from amitriptylinbe. Instead, I would go for nortriptyline because of its better safety profile and lesser anti-cholinergic side effects compared to other TCAs such as dothiepin, amitriptyline etc. HOWEVER, if you are taking a TCA for neuropathic pain management, then amitriptyline seems to be the most preferred medication despite its side effect potential. |

There is one particular group of enzymes called Cytochrome P450, which are responsible for breaking down the majority group of medications available in the market. Grapefruit juice contains an ingredient which can stop the Cytochrome P450 enzymes from metabolising the drugs. As a result, if Joe takes amitriptyline and washes it down with a cup of grapefruit juice, the enzyme's activities would come to a halt, allowing the amitriptyline level in the blood to slowly build up, causing toxicity eventually. |
