1221614 2011-08-10 03:10:00 Are you or whoever, ejecting these USB flash drives, before removing them?? Thats what Event ID: 57 may mean. If they dont it may corrupt it sooner or later. Whats this extension slot?? Is it a PCI card or an adapter / cable that plugs into a USB header on the mobo? Speedy Gonzales (78)
1221615 2011-08-10 03:13:00 Ejecting is definately the safest way, but by default windows doesn't use write caching on removable flash drives so as long as you aren't currently reading or writing to them you can theoretically just pull them out safely without ejecting.

doesn't help you with your problems of course.

1221616 2011-08-11 01:14:00 problem continuing:
I booted to Ubuntu as previously suggested, assess to all usb flash drives & the SD camera card was successful.

I pulled a PCI extension usb port & disconnected the front panel SD card reader that has 1 usb slot in addition to the SD card slots.
Each time I reboot an error is generated:

Uli PCI to USB Enhanced Host Controller Service Failed to start - specified driver is invalid.

I have removed all the usb listed devices in Device Manager - with no resolve.

1221618 2011-08-12 06:01:00 Format it, the OS the USB or the hardware is probably borked Speedy Gonzales (78)
1221619 2011-08-12 06:45:00 I guess it's likely to be a software not hardware problem, if the device works in Linux, although this is not a 100% guarantee (from my own experience)

If you've got a spare hard drive you could test again with a clean install of Windows on the new drive, before you go so far as to format and reinstall.

However that may be the best (read: quickest and easiest) way to fix the problem.
You might spend a week trying to fix up the drivers and registry entries etc and still not get it done.
