1260234 2012-02-20 20:07:00 It always amazes me when these stats come out and the public service spin doctors come out saying that they can do better and these problems are caused by process failures. . If these incidents happened at private enterprize hospitals or at work places OSH, Dept of labor and every ticket writer out there would be getting involved but as the people concern are public servants they are not accountable so nothing will change. paulw (1826)

1260238 2012-02-20 22:15:00 So the rough error rate is less than the road toll, room no doubt for improvement which is what this processes is all about. I would imagine that looking after sick people (usualy old with multiple medical problems) would be much more dangerous/complicated than driving. There are risks in many aspect of our lives.

If we want to improve things we need to know where the problems are so they can be investigated and improved/avoided, the downside is the deadlines that any report like this creates and the impression that thinigs are in a crisis. I wonder what the numbers are for the private sector.....which arent reported here

Admitting mistakes is hard and it would be devisating to discover that you injured/hurt someone as a result of an error or mistake.

Lay charges, make your arguments in court.

For this to happen once is a tragedy. More then once is a is a disgrace and shamefull.


For it to be accepted and allowed to happen is criminal.

There are ways to ensure crap like this can't be allowed to happen, this is simple stuff, and its killing people....

Have you heard of anyone winning such a case, ACC would pay probably if there was one a win.
